The Challenge

Home-based primary care is evolving fast. Delivering high-quality, value-based care in the home now requires more than clinical skill- it demands operational readiness, financial fluency, and interdisciplinary teamwork. To meet this need, the American Academy of Home Care Medicine (AAHCM) set out to create a comprehensive curriculum of over 20 modules to prepare clinicians, coordinators, and leaders for the future of home care. Each module is built around real experiences shared by home-based care experts, capturing lessons learned, best practices, and the “field wisdom” that comes only from years of doing the work.

The Process

For this project, Jackrabbit LX worked side-by-side with Academy leaders and subject matter experts to shape a learning experience that reflects the real-world complexity of home-based care. The curriculum builds applied competence and confidence through realistic decision-making scenarios, mobile-friendly design, and modules that connect the dots between patient outcomes and value-based performance.

We collaborated with physicians, nurses, social workers, therapists, administrators, and other professionals across the care continuum, transforming their expertise into actionable, practice-ready learning.

Each course emphasizes operational and financial readiness, team-based collaboration, and adaptability- equipping professionals at every level to thrive in emerging models of care.

In this short video, learning experience designer Esther Boyd shares what makes this project so distinctive: a program built from lived experience across disciplines and grounded in the realities of home-based care.

The Solution

Together, we’re building more than a curriculum, we’re building a career pathway for the home care workforce.
At Jackrabbit LX, we believe that learning design can change systems. Our passion is helping organizations turn training into transformation- scaling access, advancing professional growth, and preparing the next generation of home-based care leaders.

This program reflects what happens when experience and learning design meet, capturing the voices, challenges, and insights of the people who deliver home-based care every day.

If your organization needs to develop or scale online learning, we’d love to help.

Vital Strategies

Developing a virtual adaptation of the international non-profit Data to Policy program.

“After being disrupted by COVID, Jackrabbit LX helped us to gather insights from program participants and stakeholders to merge our short-term pivot with our longer term objectives for the program. The solution included the legacy program’s essential components – including mentoring and project-based learning – all of which have received positive feedback from participants.”
